Kelli Cremeans, DNP, APRN, FNP-C

Dr. Kelli Cremeans, DNP, APRN, FNP-C, is a board-certified Family Nurse Practitioner with over 23 years of experience in the healthcare field. She is dedicated to providing compassionate, evidence-based, and patient-centered care through a comprehensive telehealth approach.

Education and Clinical Background

Dr. Cremeans earned her Doctor of Nursing Practice (DNP) degree from Northern Kentucky University in December 2025 and holds national board certification through the American Academy of Nurse Practitioners (AANP). She previously earned her Master of Science in Nursing from the University of Kentucky in 2009.

Her diverse clinical background includes experience in primary care, stroke navigation, and infectious disease, providing a strong foundation for managing both acute and chronic conditions. Over the past several years, Dr. Cremeans has specialized in medical weight management and telehealth care, where she focuses on individualized treatment plans tailored to each patient’s unique health needs and goals.

Licensure and Clinical Background

Dr. Cremeans is currently licensed to practice in Kentucky, Colorado, and Virginia, allowing her to provide telehealth services to patients across multiple states.

Outside of her clinical practice, Dr. Cremeans is a devoted wife and mother of two children—a son and a daughter. Her commitment to health and wellness, both personally and professionally, shapes her thoughtful and compassionate approach to patient care.
